Common Wegovy Side Effects — Overview
The most common side effects of Wegovy are gastrointestinal — nausea, vomiting, diarrhoea, constipation, and abdominal discomfort. These are dose-dependent and most frequent during the dose escalation phase in the first 8–12 weeks.
In clinical trials, gastrointestinal side effects were the primary reason for treatment discontinuation, though most patients who persisted through dose escalation found symptoms manageable and often resolved. Slow, gradual dose escalation — as per the standard Wegovy schedule — reduces the intensity of early GI symptoms.
These GI side effects are not liver-related. They reflect the effect of GLP-1 receptor activation on gastric emptying and intestinal motility. They do not indicate liver damage.
The Liver-Specific Side Effect Picture
Wegovy has a favourable liver safety profile. In the ESSENCE trial — the phase 3 study of semaglutide 2.4mg in patients with established MASH and liver fibrosis — there were no treatment discontinuations due to liver enzyme elevations across the entire semaglutide treatment arm.
This is significant because ESSENCE enrolled patients who already had significant liver disease — the population most vulnerable to drug-induced liver injury. The absence of liver-related discontinuations in this trial is the strongest available evidence that Wegovy, at therapeutic doses, does not cause liver toxicity.
AASLD practice guidance updated in November 2025 now includes semaglutide as a therapeutic option for MASH with F2–F3 fibrosis — a recommendation that would not be made for a medication with significant hepatic toxicity.
Temporary Liver Enzyme Elevations During Rapid Weight Loss
Some patients on Wegovy experience a transient rise in ALT and AST during the early months of treatment — particularly when weight loss exceeds 1–1.5kg per week.
The mechanism: rapid fat mobilisation from body stores increases the delivery of free fatty acids to the liver for processing. During periods of very rapid weight loss, this can temporarily overwhelm the liver's processing capacity, producing a short-term enzyme elevation.
This is not drug-induced liver injury. It is a physiological response to the rate of fat metabolism — the same phenomenon seen with very low calorie diets and bariatric surgery. The elevation is self-limiting. As weight loss stabilises, enzymes typically return to normal or continue improving as overall liver fat reduces.
Rare Drug-Induced Liver Injury — What to Know
While the overall safety profile is favourable, rare cases of drug-induced liver injury associated with semaglutide have been reported in the medical literature. A case report published in September 2025 documented semaglutide-associated liver injury in a patient who developed jaundice and significantly elevated liver enzymes after starting treatment.
These cases are rare — they do not represent the typical experience on Wegovy. But they are real, and they underscore why monitoring matters. Symptoms that should prompt urgent medical review include:
- Jaundice (yellowing of skin or eyes)
- Dark urine
- Pale stools
- Persistent severe right upper abdominal pain
- Unexplained fatigue with rapidly rising liver enzymes
Gallbladder Disease — The Most Clinically Significant Liver-Related Complication
Gallbladder disease is the most clinically significant complication of Wegovy in the context of liver health — not because it damages the liver directly, but because gallstones and bile duct obstruction can produce liver enzyme elevations that may be confused with liver disease progression.
A meta-analysis of 76 GLP-1 studies found a 27% increased risk of gallstones and 36% increased risk of gallbladder inflammation compared to non-GLP-1 users. The highest risk period is the first 6–12 months when weight loss is most rapid.
Symptoms of gallbladder problems on Wegovy: right upper abdominal pain particularly after eating, nausea, pain radiating to the right shoulder blade. Severe persistent pain with fever or jaundice requires same-day medical review.
Thyroid Tumour Warning — Not Liver-Related but Important
Wegovy carries a boxed warning about thyroid C-cell tumours based on rodent studies. Semaglutide caused thyroid C-cell tumours in rats and mice at clinically relevant exposures. It is unknown whether Wegovy causes thyroid C-cell tumours, including medullary thyroid carcinoma, in humans.
Wegovy is contraindicated in patients with a personal or family history of medullary thyroid carcinoma or Multiple Endocrine Neoplasia syndrome type 2. This warning is not liver-related but is part of the complete safety picture patients should understand before starting treatment.
Recommended Liver Monitoring on Wegovy
For patients on Wegovy, the following liver monitoring schedule is reasonable:
Before starting: Full liver function panel (ALT, AST, GGT, ALP, bilirubin, albumin, platelets). FIB-4 calculation. Liver elastography if FIB-4 is indeterminate.
At 3 months: Repeat ALT and AST to assess for transient enzyme changes during rapid weight loss.
At 12 months: Repeat full liver function panel and FIB-4. Consider repeat elastography if baseline showed elevated stiffness.
Annually thereafter: Liver function panel. Elastography every 1–2 years for patients with known MASLD or MASH.
