Elevated Liver Enzymes on Ozempic — What It Means

If your liver function test came back with elevated ALT, AST or GGT while on Ozempic, you're not alone — and it's usually not a reason for alarm. Here's what each enzyme measures, what normal values are, and when to take it seriously.

What each enzyme measures

  • ALT (alanine aminotransferase): a liver-specific enzyme; rises when liver cells are stressed.
  • AST (aspartate aminotransferase): rises in liver cell damage but also from muscle damage.
  • GGT (gamma-glutamyl transferase): rises with bile duct stress or alcohol.
  • ALP (alkaline phosphatase): bone and bile origin; rises in bile duct obstruction.
  • Bilirubin: waste product; high levels cause jaundice.

ALT is the most useful single number for fatty liver and treatment response. A consistent reduction in ALT over 6–12 months on Ozempic suggests improving liver health.

What typically happens to enzymes on Ozempic

  • Months 1–3: small, transient enzyme rises in some patients
  • Months 3–6: consistent reduction in ALT/AST as liver fat reduces
  • Months 6–12: most patients return to normal range or remain in mild elevation
  • Long-term: stable or further-improving values in most patients

When elevated enzymes need investigation

  • ALT or AST greater than 3× the upper limit of normal
  • Persistent elevation beyond 3–6 months
  • Associated symptoms — jaundice, abdominal pain, fatigue, dark urine
  • Rising trend rather than falling on serial tests
  • Any elevation of bilirubin

What to do if your enzymes are elevated

  1. Don't stop Ozempic without discussing with your GP
  2. Repeat LFTs after 2–4 weeks
  3. If still elevated, your GP will assess: alcohol, other medications, viral hepatitis, gallbladder issues, autoimmune disease
  4. Elastography to stage liver fibrosis if not already done
  5. Specialist referral if persistent unexplained elevation

What's a normal ALT on Ozempic?

ALT below 35 U/L (women) and 50 U/L (men) is typically considered normal. Many patients on Ozempic see ALT fall over 6–12 months as liver fat reduces. Transient mild elevations in the first 3 months are not unusual.

Should I stop Ozempic if my liver enzymes are elevated?

No — don't stop without discussing with your GP. Most mild enzyme elevations resolve. Genuine drug-induced liver injury from semaglutide is rare and presents with much higher elevations plus symptoms.

What does an elevated GGT on Ozempic mean?

GGT rises with bile duct stress, fatty liver, alcohol, and many medications. Mild GGT elevation in fatty liver is common; persistent isolated GGT elevation should be investigated for other causes.

How long until my liver enzymes improve on Ozempic?

Most patients see consistent reduction in ALT/AST by 3–6 months. Significant improvement is usually visible by 6 months and continues through 12 months as weight loss and liver fat reduction accumulate.
