- Patient ID / MRN
- Date and time of examination
- Operator and site
- Clinical indication (MAFLD, HBV, HCV, indeterminate FIB-4, etc.)
- Median kPa (E median)
- IQR and IQR/median ratio
- Number of valid acquisitions (e.g. 10/10)
- Reliability statement (IQR/M ≤30% = reliable)
- Fibrosis stage interpretation (disease-specific)
- UAP or CAP value and unit
- Steatosis grade S0–S3
- Note if CAP vs UAP — grades comparable, raw dB/m not
- Representative B-mode image(s) showing measurement zone
- Probe position / intercostal space (guided systems)
- Exclusion of vessels and gallbladder shadow where relevant

What must be on a liver elastography report?
At minimum: median liver stiffness (kPa), measurement reliability (IQR/median), steatosis output (CAP or UAP with grade), number of valid shots, clinical indication, and operator/site identification. Guided systems should include a representative ultrasound image confirming measurement location.
Is there a standard FibroScan report format in Australia?
FibroScan devices export manufacturer-formatted reports. Clinics using guided 2D-SWE or integrated ultrasound elastography often need a custom template aligned with RANZCR-style ultrasound reporting and GP referral expectations. This checklist adapts to both.
